After you register online to use the Claim and Eligibility Look-Up, simply enter your Provider Tax ID and password to access your secure claims records and patient's eligibility/benefits. You can view the status for submitted claims and your patient's individual and family deductible amounts, and out-of-pocket amounts that have been satisfied.

For those times when you can't access the Internet, you can receive claims information over the telephone anytime by calling the toll-free number on the back of the patient's Medical Identification Card.
